Management is usually conservative: appropriate medication, graded activity and exercise, posture and ergonomics, and patient education. For nerve-root symptoms, structured therapy and sometimes injections are considered. Surgical referral is reserved for specific conditions such as significant or progressive nerve compression.
Note: Back or neck pain accompanied by loss of bladder or bowel control, severe progressive weakness, or high fever requires urgent medical attention.

For the large majority of back pain, surgery is not needed. Treatment focuses on medication, physical therapy, posture and lifestyle changes, with surgery reserved for specific nerve-related problems.
Muscle pain is usually felt locally and often responds to rest and therapy. Nerve-related pain tends to radiate down the leg or arm, and may include numbness or tingling. An examination helps distinguish the two.
Seek prompt attention if back pain is accompanied by loss of bladder or bowel control, progressive weakness or numbness in the legs, or follows a serious injury.
